29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) Residential construction, side wall: On or about July 24, 2025, the employer exposed employees to ejection/fall hazards of approximately 14-feet while working from the Genie S-80J Aerial Lift, in that a fall arrest system was not used by employees working in the basket as required.
