A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a) 2530 Slater Mill Rd Douglasville. Trench: On or about July 24, 2025, the employer exposed employees to cave-in hazards in that they allowed employees to perform work installing a sewer line (pipe) in a trench 36ft long, 11ft wide and 4.6 +ft deep without a ladder or other safe means of ingress or egress.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: Trench operation located at 2530 Slater Mill Rd Douglasville GA. 30135. On or about July 24, 2025, the employer exposed employees to cave-in/entrapment hazards in that the spoil pile and excavation equipment were not kept at least 2ft from the edge of the excavation while employees worked in the trench that measured an estimated 36ft long by 11ft wide by 4.6ft deep.
