Worksite: 19 South 23rd Street, Wyandanch, NY 11798. On or about 07/23/2025, an employee was installing shingles on a two-story residential building. The employee was not utilizing a means of fall protection and was exposed to falling approximately 22 ft to the ground below. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
