29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite: 137 E Cumberland St, Philadelphia - On or about July 25, 2025, the employer did not provide effective training for each employee conducting framing activities from the 2nd level of a 3 story residential apartment building, and exposed to fall hazards of approximately 17 feet, in the recognition and avoidance of fall hazards.

29 CFR  1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:  a) Jobsite: 137 E Cumberland St, Philadelphia - On or about July 25, 2025, the employer did not initiate or maintain an accident prevention program to address the associated hazards that employees are exposed to during framing activities, such as but not limited to fall hazards.  The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1764612, citation number 1, item number 1, and was affirmed as a final order on 2/5/2025, with respect to a workplace located at 103 North 63rd Street, PHILADELPHIA, PA 19149.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b):  a) Jobsite: 137 E Cumberland St, Philadelphia - On or about July 25, 2025, the employer did not ensure that employees conducting framing activities from  2nd level of a 3 story residential apartment building, approximately 17 feet above the ground level, were protected from fall hazards by the use of a fall protection system.  The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1764612, citation number 1, item number 2, and was affirmed as a final order on 2/5/2025, with respect to a workplace located at 103 North 63rd Street, PHILADELPHIA, PA 19149.
