29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). a) Core and Main DBA Enviroscape EMC, LTD on or about July 25, 2025, at the location of 300 South Chestnut Street Deshler, Ohio: The employer failed to ensure employee tasked with clearing jams from the Beater Drums on Line 1-4 were adequately protected from unexpected reenergization of the equipment, exposing them to caught in hazards. b) Core and Main DBA Enviroscape EMC, LTD on or about July 25, 2025, at the location of 300 South Chestnut Street Deshler, Ohio: The employer failed to ensure employee tasked with correcting net tears atop Draw Rollers on Lines 1-4 were adequately protected from unexpected reenergization of the equipment, exposing them to lacerations and caught in hazards.
