(a) Mile Post 36/6 Eastbound, Atlantic City Expressway, Winslow, NJ: On or about 07/25/25 an employee was exposed to struck-by hazards involving a backing Stake Bed Truck with its Truck Mounted Attenuator lowered, while in the process of removing traffic cones set in a closed shoulder of the Atlantic City Expressway. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a- Mile Post 36/6 Eastbound, Atlantic City Expressway, Winslow, NJ: An employee assigned to remove traffic cones from a shoulder, while working and walking next to and near live motor vehicle traffic and a backing Truck Mounted Attenuator, was not protected from struck-by hazards when the employer did not adequately provide the employees with training regarding traffic safety awareness, including recognition and avoidance of back over hazards. Condition observed or or about 07/25/25. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
