29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation  (a)  On or about July 25, 2025, at a construction site located at W327S6718 Fox court, Mukwonago, WI 53149, an employee was operating a pneumatic nail gun without eye protection while installing shingles on the north side of the second story roof, exposing the employee to a struck by / eye injury hazard.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  (a) On or about July 25, 2025, at a construction site located W327S6718 Fox court, Mukwonago, WI 53149, three (3) employees were performing roof installation work on the second story of a residential roof with a 6:12 pitch, were not using fall protection. This exposed employees to a fall hazard of approximately 17 feet from roof to the ground below.  (b) On or about July 25, 2025, at a construction site located W327S6718 Fox court, Mukwonago, WI 53149, three (3) employees were performing roof tear off work on the south side of the first story of a residential roof with a 6:12 pitch, were not using fall protection. This exposed employees to a fall hazard of approximately eight (8) feet from the roof to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about July 25, 2025, at a construction site located W327S6718 Fox court, Mukwonago, WI 53149, employees installing shingles and performing tear off work on an existing residential home, used two (2) Gorilla Aluminum ladders on the west side of the home to access the first story of the roof that was not erected three (3) feet above the upper landing or secured in place.

29 CFR  1926.1053(b)(6): Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement.  (a) On or about July 25, 2025, at a construction site located W327S6718 Fox court, Mukwonago, WI 53149, the ladder on the first story reaching up to the second story of the roof was not on a stable surface and was not secured in place, in-that the ladder was the top section of an extension ladder with no cleats, that was placed on the apex of the first story roof, and leaned up against the chimney without being secured into place to prevent displacement. This exposed employees to a fall hazard off the ladder/roof to the ground below.
