OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to struck-by hazards.  On or about July 28, 2025, at 4187 Erie Street, Willoughby, OH 44094, an employee was exposed to struck by hazards while performing maintenance work on a historical clock. The employee was working from an elevated vehicle mounted aerial bucket and employer provided cones were not utilized while work was being performed as trained. The knuckle of the boom extended into traffic which the employee was not protected from through employer provided cones.
