Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:   On or about July 25, 2025, Lemus Contractor LLC did not ensure employees were protected from fall hazards while performing roofing work at heights ranging from 10 to 24 feet above lower levels at a worksite located at 6023 Wagon Mount Court in Saint Louis, Missouri.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:    On or about July 25, 2025, Lemus Contractor LLC did not ensure a Werner extension ladder was properly positioned so the side rails extended three feet past the upper landing surface, exposing employees to fall hazards at a worksite located at 6023 Wagon Mount Court in Saint Louis, Missouri.
