29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns. (a) On or about July 28, 2025, employees were performing clean up on the ground without a hard hat on, while other employees were installing shingles and throwing debris from the roof above, exposing the employees to a struck-by hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about July 25, 2025, the employer had not ensured that employees performing clean up and shingle installation on the main roof of the residential property were protected from falling to the lower level, exposing employees to a fall hazard of approximately 19 feet to the ground below. (b) On or about July 25, 2025, the employer had not ensured that employees performing clean up and shingle installation on the lower roof (over front door) were protected from falling to the lower level, exposing employees to a fall hazard of approximately 10 feet to the ground below.
