29 CFR  1910.95(c)(1):The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  On September 2, 2025, an employee was exposed to an 8-hour time weighted average noise level of 102.5 dBA (equivalently a dose of 571.7%) in excess of the 85 dBA (equivalently a dose of 50%) while washing parts with an air hose, where a Hearing Conservation Program was not fully implemented including monitoring, annual audiometric testing program, noise training, and the posting of the noise standard 1910.95.

Location: Wet Spray  On or about September 2, 2025, employees were exposed to fire and burn hazards where a 55 gallon drum of 99% isopropanol, a Category 2 flammable liquid with a flashpoint of 54 °F, was being dispensed into containers where the nozzle and container were not electrically interconnected.

Location: Powder Coating  On or about July 28, 2025, employees were exposed to corrosive materials that could cause serious eye damage while working with chemicals such as Gardobond R2606 and POWDURA TGIC Polyester Powder Coating, where there were not suitable facilities for flushing of the eyes within the work area.
