Naval Base San Diego, Maintenance Department, building A, B, C and Palmar Hall: On or about July 28, 2025 and at times prior to, the employees conducted various maintenance tasks on a regular basis without having a written hazard assessment established for the facilities. This condition(s) potentially exposed employees to physical and chemical hazards.

29 CFR 1910.134(c)(2)(i):Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible. Naval Base San Diego, Maintenance Department, building A, B, C and Palmar Hall: On or about July 28, 2025, and at times prior to, the employees utilized respirators Honeywell 5500 half face respirator and 3M 85011 NIOSH N95 respirators, during the maintenance tasks conducted in the Naval Base San Diego 32nd street. A copy of the Appendix D was not provided which potentially exposed employees to respiratory hazards.
