29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a) Jobsite, 107 Wells Lane, Derry, PA:  On or about July 24, 2025,  seven employees were installing shingles on a residential roof at heights up to 12.5 feet without utilizing fall protection.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) Jobsite, 107 Wells Lane, Derry, PA:  On or about July 24, 2025, employees were working at heights up to 12.5 feet above the ground without fall protection and the owner did  not provide documentation that the employees were trained in fall protection.
