29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative   Location:  Facility  Instance A:   Company employees were not locking out machinery such as power presses prior to performing servicing tasks such as die setting.  Instance B:   The company was not performing inspections of LOTO procedures at least annually.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Location:  Press Rooms  Instance A:  The Dallas Industries coil straightener for the # 14 Bliss C-150 press was unguarded. Instance B:  Coil unwinding mandrels were unguarded - including mandrels serving Presses 14, 17, 28 and 33. Instance C:  VMX 50 vertical milling machine's front door interlock was inoperable. Instance D:  the lowermost of 3 chain and sprockets on the # 39 V & O Press was unguarded.
