29 CFR  1926.451(b)(1):Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is not planked  On or about  July 24, 2025, adjacent entrance to building on fourth tier of scaffold, employees utilized a scaffold that was not fully planked while performing stucco work on the exterior of the building.

29 CFR  1926.451(e)(1):When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  On or about July 24, 2025, adjacent entrance of building, employees utilized a scaffold without ladders for access and egress while performing stucco work on the exterior of the building.

29 CFR  1926.451(g)(4)(i):29 CFR 1926.451(g)(4)(i):  Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews.  On or about  July 24, 2025, adjacent entrance to building on fourth tier of scaffold, employees utilized a scaffold with missing end guardrail systems while performing stucco work on the exterior of the building.

29 CFR  1926.453(b)(2)(v):A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  On or about July 24, 2025, on the southwest side of building, an employee utilized a Genie aerial lift without fall protection while performing stucco work on the exterior of the building.
