29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use: a) For the cylinder storage area at , 6012 31st Street East, Bradenton, FL, on or about 07/29/2025, the employer exposed employees to projectile struck-by hazards, in that, a compressed gas cylinder in storage was missing valve protection cap.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: a) 6012 31st Street East, Bradenton, FL 34203; On or about 7/29/25, in the front building in the welding area, employees were exposed to fire and electrical hazards, in that, an electrical panel was blocked by a full trashcan and wood debris on the floor, preventing access to it in case of emergency. The minimum clearance of 36 inches was not maintained. b) 6012 31st Street East, Bradenton, FL 34203; On or about 7/29/25, in the front building in the welding area, employees were exposed to fire and electrical hazards, in that, an electrical panel was blocked by a a table with tools, toolbox and PPE, preventing access to it in case of emergency. The minimum clearance of 36 inches was not maintained.
