29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 7/29/2024, the employer did not protect employees who were engaged in residential roofing operations 6ft (1.8m) or more above lower levels while at the job site located at 3189 Lupine Dr. in Arnold MO. The employees were conducting residential roofing operations and were exposed to the hazard of falling greater than ten (10) feet.   Juan Ortiz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1707376, citation number 1, item number 4 and was affirmed as a final order on 12/12/2023, with respect to a workplace located at 538 Forest Park Drive in Wentzville, MO.  Juan Ortiz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1707444, citation number 1, item number 1 and was affirmed as a final order on 12/12/2023, with respect to a workplace located at 544 Forest Park Drive in Foristell, MO.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:   On or about 7/29/2025, the employer did not provide a training program for each employee who was exposed to fall hazards while roofing a residential building located at 3184 Lupine Drive in Arnold, MO.  The employees were not trained in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. The employees were conducting roofing operations that exposed them to the hazard of falling from a height of greater than ten (10) feet.  Juan Ortiz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.503(a)(1), which was contained in OSHA inspection number 1707444, citation number 1, item number 2 and was affirmed as a final order on 12/12/2023, with respect to a workplace located at 544 Forest Park Drive in Foristell, MO.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On 7/29/2025, the employer did not ensure portable ladders were constructed and placed so three (3) feet or more. Workers performing residential construction were exposed to falling great than ten (10) feet while using a straight ladder to ascend/descend from the ground level to the roof at the job site located at 3189 Lupine Drive in Arnold, MO.
