29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   b) 11341 Colony Cir, Broomfield, CO 80021: On or about July 28, 2025, and at times prior, an employee performing siding operations and building a pump jack scaffold was exposed to fall hazards greater than 6 feet while accessing the scaffold by climbing onto it from a balcony railing of the second level. The employer failed to ensure employees were provided a safe means of access to reach the scaffold during assembly and work.

29 CFR  1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:  a)  On or about July 28, 2025, employees were exposed to fall hazards while working from a pump jack scaffold in that poles were not secured to the structure at intermediate points along the pole as necessary.  Poles in excess of 30 ft in length were in use with only the top brace in use.  The scaffold poles and platform moved excessively during use.

29 CFR  1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.   a)  On or about July 28, 2025, employees erecting pump jack scaffolding at the jobsite were not providing with training on proper assembly, access and hazards associated with working from an elevated scaffold.

The employer failed to ensure employees were protected from falls by utilizing a fall protection system or guardrails.  b) 11341 Colony Cir, Broomfield, CO 80021: On or about July 28, 2025, and at times prior, an employee performing siding operations and building a pump jack scaffold was exposed to fall hazards greater than 6 feet while standing on the balcony railing of the second level patio. The employer failed to ensure employees were protected from falls by utilizing a fall protection system or guardrails.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  On or about July 28, 2025, employees were exposed to falls in excess of 20 ft while using an extension ladder to access a second-story roof that did not extend at least 3 ft above the roof surface.
