29 CFR 1910.119(j)(2): The employer did not establish written procedures to maintain the on-going integrity of process equipment. (a) On July 28, 2025, at 2841 3rd Street SW, Underwood, North Dakota, the employer did not establish a complete written procedure to maintain the ongoing integrity of VOP-7. The procedure provided to employees addressed only oil draining and did not encompass the full scope of the process, which included draining both oil and ammonia.

29 CFR 1910.119(j)(3): The employer did not train each employee involved in maintaining the on-going integrity of process equipment in an overview of that process and its hazards and in the procedures applicable to the employee's job tasks to assure that the employee can perform the job tasks in a safe manner. (a) On July 28th, 2025 and at times prior to, at 2841 3rd Street SW Underwood, North Dakota, the employer did not provide employees with training on how to safely perform the task of draining ammonia and oil from process equipment.
