29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) On or about July 30, 2025, at the worksite, 101 Village Blvd. S, Baldwinsville NY 13027: Employees were picking roofing debris off the ground while work was being done on the roof above them exposing the employees to a head injury hazard.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about July 30, 2025, at the worksite, 101 Village Blvd. S, Baldwinsville NY 13027: Employees were installing roofing material on an estimated 5:12 pitch roof  with an approximate ground to eave height of 16 feet without fall protection of any kind exposing the employees to a fall hazard greater than 6 feet.
