29 CFR 1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s) a) Good Samaritan Ministries Back Alley: On or about July 29th, 2025, and at times prior, employees were exposed to struck by hazards while operating a Toyota forklift (model number: 3FG10-10486) in the back alley of the facility. The forklift did not have lights to illuminate obstructions and employees. b) Good Samaritan Ministries Back Alley: On or about July 29th, 2025, and at times prior, employees were exposed to struck by hazards while operating a Toyota forklift (model number: 3FG10-10486) in the back alley of the facility. The forklift did not have a horn to warn employees a forklift is near and being operated. c) Good Samaritan Ministries Back Alley: On or about July 29th, 2025, and at times prior, employees were exposed to struck by hazards while operating a Toyota forklift (model number: 3FG10-10486) in the back alley of the facility. The forklift did not have a backup alarm to warn employees a forklift is near and being operated.
