Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):  a)  On or about July 31, 2025, employees using the shrink wrapping equipment in Building  B were exposed to electrical hazards and the employer had not examined the equipment to ensure that extension cords that were series connected (daisy chained) were used in accordance with their label and listing.  b) On or about July 31, 2025, employees working near the NW corner of Building B were exposed to electrical hazards and the employer had not examined the cord(s) to ensure that flexible cords were only used in continuous lengths.  c) On or about July 31, 2025, employees working in the NW corner of  Building B were exposed to electrical hazards and the employer had not examined the equipment to ensure that extension cords and relocatable power taps (RPT) that were series connected (daisy chained) were used in accordance with their label and listing.
