29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  a) Jobsite located at 12 Langton Lane in Newtown Square, Pennsylvania- On or about July 31, 2025, the employer did not remove worn or frayed electrical cords that were damaged and in an unsafe condition from the jobsite.  Electrical cords had damaged outer insulation paired, thereby exposing employees to electrical hazards.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) Jobsite located at 12 Langton Lane in Newtown Square, Pennsylvania- On or about July 31, 2025,  the employer did not ensure that employees conducting gutter installation activities were provided fall protection systems or guard rail systems, thereby exposing employees to fall hazards of approximately 35 feet.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   a) Jobsite located at 12 Langton Lane in Newtown Square, Pennsylvania- On or about July 31, 2025, the employer did not train each employee conducting gutter installation activities on scaffolds, who were exposed to fall hazards between approximately 35 feet, to recognize the hazards associated with working from scaffolds to understand the procedures to control or minimize those hazards.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Jobsite located at 12 Langton Lane in Newtown Square, Pennsylvania- On or about July 31, 2025,  the employer did not ensure an employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net system, or personal fall arrest system.  An employee was performing caulking of the residential building approximately 13 feet to the ground level without the use of a fall protection system.

Employees performed gutter installation and caulking activities without being trained on fall protection, thereby exposing employees to fall hazards.

29 CFR  1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed.  a) Jobsite located at or near 1388 S 10th Street, Philadelphia PA- Exterior Scaffold- On or about April 22, 2025, the employer did not ensure an employee working from an  A- frame ladder were using the ladder in which it was designed. The employee used  a closed A- frame ladder, propped against the wall of the building exposing employees to fall hazards.
