29 CFR 1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) Midwest Welding & Machine: On or about July 30th, 2025, and at times priors, employees were exposed to chemical hazards from powder coating sprays while wearing a tight fitting 3M elastomeric half-mask respirators to perform powder coating onto selected metal materials. The employer failed to ensure that employees were medically evaluated prior to respirator use. b) Midwest Welding & Machine: On or about July 30th, 2025, and at times priors, employees were exposed to chemical hazards from powder coating sprays while wearing a tight fitting 3M elastomeric half-mask respirators to perform powder coating onto selected metal materials. The employer failed to ensure employees were fit tested prior to respirator use. c) Midwest Welding & Machine: On or about July 30th, 2025, and at times priors, employees were exposed to chemical hazards from powder coating sprays while wearing a tight fitting 3M elastomeric half-mask respirators to perform powder coating onto selected metal materials. The employer failed to ensure a respiratory maintenance program was implemented. d) Midwest Welding & Machine: On or about July 30th, 2025, and at times priors, employees were exposed to chemical hazards from powder coating sprays while wearing a tight fitting 3M elastomeric half-mask respirators to perform powder coating onto selected metal materials. The employer failed to ensure that employees were trained on respirator use and limitations prior to use.
