29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: On or about July 24, 2025, in the Shop, employees welding and grinding steel worked near continuous noise without a Audiometric Testing Program at the following levels: a) An employee was exposed to continuous noise at a level of 95.9 dBA during a sampling period of 504 minutes. b) An employee was exposed to continuous noise at a level of 100.8 dBA during a sampling period of 501 minutes.

29 CFR 1910.252(c)(1)(ii): When welding had to be performed in a space entirely screened on all sides, the screens were not so arranged that no serious restriction of ventilation exited and to protect nearby workers from the glare of welding: a) On or about July 24, 2025, at the Shop, employees working and walking in open spaces were not protected from ultraviolet rays emitted from nearby welding operations.
