29 CFR 1910.303(b)(1):Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1): a) 105 South Buttonwood Street Fleetwood, PA: On or about July 31, 2025, the electrical safety interlock switches on six B-Line Filler plexiglass guards ceased to function, which failed to prevent the operation of the bottling carousel when the guards were open, exposing employees to a rotating parts hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1904.41(a)(2): The employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illness to OSHA or OSHA's designee when their establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and their establishment was classified in an industry listed in appendix A to subpart E of this part. 105 South Buttonwood Street Fleetwood, PA: On or about the August 12, 2025, the employer did not submit their OSHA 300 data for calendar year 2024 to the Injury Tracking Application prior to the required deadline, on or about March 2, 2025. a) The establishment employed 119 employees and was classified in North American Industry Classification System (NAICS) 312111 during calendar year 2024. No abatement certification or documentation required.
