29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury.  On or about July 31, 2025, and times prior thereto, in the backyard of the residential buildings, employees were performing utility installation work activities and were not trained to recognize trenching/excavation hazards.

29 CFR  1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means.  On or about July 31, 2025, and times prior thereto, in the backyard of the residential building, the employer did not use safe and acceptable means to locate an underground utility, and a gas line was struck.

29 CFR  1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  On or about July 31, 2025, and at times prior thereto, in the backyard of residential buildings, employees engaged in utility installation work in excavations with excavated material (spoils) not kept at least two (2) feet from the edge of the excavations nor retained by a device capable of preventing materials from falling or rolling into the excavation.

29 CFR  1926.652(c)(2)(iii): Manufacturer's specifications, recommendations, and limitations, and manufacturer's approval to deviate from the specifications, recommendations, and limitations were not in written form at the jobsite during construction.  On or about July 31, 2025, and times prior thereto, in the backyard of the residential buildings, employees were engaged in utility installation work in an excavation with shoring not being used according to the manufacturer specifications.

29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  On or about July 31, 2025, and times prior thereto, in the backyard of the residential buildings, employees were engaged in utility installation work in excavations of more than five (5) feet with inadequate protective systems.  The T-G Excavating Inc. was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, which was contained in OSHA inspection number 1611473, citation number 1, item number 3, and was affirmed as a final order on 01/05/2023, with respect to a workplace located at South 57th and 21st in Tulsa, OK.
