29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) Jobsite located at or near 812 McKeen St. Philadelphia, PA, 19148. The employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and guardrail supports. Two employees conducting masonry activities were utilizing a scaffold platform on the front of the building that was not fully planked, thereby exposing employees to fall hazards of approximately 17 feet.  b) Jobsite located at or near 812 McKeen St. Philadelphia, PA, 19148. The employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and guardrail supports. One employee was conducting masonry activities, utilizing a scaffold platform on the side of the building that was not fully planked, thereby exposing employees to fall hazards of approximately 17 feet. 29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  The scaffold frame was used as a means of access.   a) Jobsite located at 812 McKeen St. Philadelphia, PA, 19148. The employer allows employees to use the scaffold frame on a tubular welded frame scaffold as means of access. Two employees were climbing the scaffold frame to access the front scaffold to conduct masonry activities. Employees were exposed to a fall hazard of approximately 17 feet in height to the ground level below.  b) Jobsite located at 812 McKeen St. Philadelphia, PA, 19148. The employer allows employees to use the scaffold frame on a tubular welded frame scaffold as means of access. One employee was climbing the scaffold frame to access the side scaffold to conduct masonry activities. The employee was exposed to a fall hazard of approximately 17 feet in height to the ground level below.  Condition was noted on or about July 31, 2025.

The employer did not ensure each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.    a) Jobsite located at 812 McKeen St. Philadelphia, PA, 19148. The employer did not ensure each employee on a scaffold more than 10 feet above a lower level was protected from falling to that lower level. Two employees engaged in masonry activities were utilizing a scaffold platform located on the front of the building. The scaffold did not have guardrail systems along the open sides and ends of the working platform. The employees did not utilize fall protection, thereby exposing employees to fall hazards of approximately 17 feet.  b) Jobsite located at 812 McKeen St. Philadelphia, PA, 19148. The employer did not ensure each employee on a scaffold more than 10 feet above a lower level was protected from falling to that lower level. One employees engaged in masonry activities was utilizing a scaffold platform on the side of the building.
