29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  (a) On or about August 1, 2025, the employer had not ensured frequent and regular inspections of the job site were conducted by a competent person to ensure hazardous conditions were found and corrected. Hazards identified included fall hazards, eye hazards, and struck-by hazards.

29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  (a) On or about August 1, 2025, employees were performing clean up on the ground and bringing roofing materials to the roof without a hard hat on while other employees were installing shingles on the roof above, exposing the employees head to a struck-by hazard.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).   (a) On or about August 1, 2025,  the employer did not ensure that employees performing tear off of a two-story residential home roof were protected from falling to the ground below. This exposed the employees to a fall hazard of approximately 10 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  (a) On or about August 1, 2025, the employer did not ensure the extension ladder used by employees to access and egress the residential roof was extended at least three feet above the upper landing or secured to prevent displacement of the ladder, exposing employees to a fall hazard of approximately 10 feet to the ground below.
