29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check: (a) On and before August 1, 2025, the employer did not ensure that employees were protected from fire hazards in that two portable fire extinguishers inside the facility had expired annual inspections. One fire extinguisher was last inspected in April 2024 and the other was last inspected in May 2023. (b) On and before August 1, 2025, the employer did not ensure that employees were protected from potential fire hazards in that the fire extinguisher mounted onto the Yale Lift Truck, Unit 92 was last inspected in October 2021.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck: On or before August 1, 2025, untrained employees were authorized to operate the company's Yale Lift truck, unit 92 when loading/unloading trailers, and moving inventory thus potentially exposing the operator and other employees in the immediate area to struck-by and/or crushing injuries.
