29 CFR  1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  Welding Shop: On or about July 05, 2024, and sometime thereafter, compressed gas cylinders of various content and size, were not properly secured and/or utilized their protective caps for valve protection thereby ensuring that the compressed gas tanks were properly secured and protected from damage in accordance with the following Compressed Gas Association Pamphlet P-1-1965 paragraph 3 subsections:  3.1.8 Each tank stored in the gas cylinder storage cabinet was not properly labeled w/ICC (Interstate Commerce Commission) label. 3.2.1 Full acetylene gas cylinders caps stored in the gas tank cabinet were not placed on the tanks at all times when they were not in use.  3.3.3 Tanks must be stored together regarding contents. Flammable gases cannot be stored with oxidizing gases 3.3.4 Full and Empty tanks must be stored separately 3.3.6 Not to be stored near highly flammable substances like oil, gas, or waste.  3.3.8 Tanks must be protected from potential impact damage. Tanks must have caps when stored. 3.3.7 / 3.3.9 Tanks should not be stored directly on open ground or exposed to rust & corrosive materials 3.3.10 Tanks should be protected from tampering by others 3.4.3 Caps must remain on tanks unless being used 3.4.4 Tanks must be properly supported 3.5.3 Cannot store flammable gases w/oxygen (green) tanks

29 CFR  1910.252(a)(2)(xiii)(C): The employer did not recognize its responsibility for the safe usage of cutting or welding equipment, and did not insist that cutters or welders and their supervisors were suitably trained in the safe operation of their equipment.  Welding Shop: On or about July 05, 2024, and sometime thereafter, the employer did not ensure that cutters and welders or their supervisors were suitably trained in the safe operation of their equipment when an oxygen-fuel gas welding or cutting system was used by more than one employee to perform TIG and MIG welding on carbon and stainless steel parts.
