29 CFR 1926.20(b)(2):  Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  Location: 50 Reservoir Street, Mansfield, MA  On or about 7-31-2025 an employees were exposed to fall hazards of more than 10 & 30 feet from:  Instance #1:  a workbox on a telescoping fork truck, to the ground below, where personal fall protection was not properly used.  Instance #2: Unguarded 3rd floor wall openings, causing employee exposure to fall hazards  The PATRIOTS CONSTRUCTION SERVICES INC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1713140, citation number 1, item number 3 and was affirmed as a final order on 04-17-2024, with respect to a workplace located at 47 Ledge Hill Road, West Roxbury, MA.
