29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a)Amerect, Inc. at 1300 11th Street Southeast, Watertown, SD 57201: On or about and at times prior to July 31, 2025, the employer did not ensure employees were protected from fall hazards while installing railing on an empty water holding tank, in that the ladder used to access the top surface of the tank wall was not extended at least 3 feet from the top wall surface. This condition exposed employees to fall hazards greater than 18 feet.
