Location:  377 Research Parkway Meriden, CT 06450  On or about August 5, 2025, and times prior to, the employer exposed employees to falls 4 feet or more above a lower level while working on a Raymond Gofer Model 233-OPC22T.

29 CFR1910.178(a)(4):  Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  Location:  377 Research Parkway Meriden, CT 06450  On or about August 5, 2025, and times prior to, the employer modified the use of a HELI CPD25 forklift affecting capacity and safe operation without the manufacturer's prior written approval by use of a fork mounted drum lifter to the forks of the forklift.  Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly.

29 CFR1904.41(a)(1)(i): If your establishment had 20-249 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.  Location:  377 Research Parkway Meriden, CT 06450  On or about August 5, 2025, the employer controlling 49 employees and operating under NAICS Code - 325998 All Other Miscellaneous Chemical Product and Preparation Manufacturing, had not electronically submitted the OSHA 300A Annual Summary data for calendar year 2024 which was required by March 2, 2025.
