The employees were exposed to a fall hazard when performing masonry work, approximately thirty-six (36) feet above the ground, from the end of a platform that extended over its support approximately seven (7) feet. The platform was not installed with a fall protection system, and it was not designed or installed in a way that allows the cantilevered portion to support employees without tipping.

Employees were observed ascending and descending from the ladders that supported the scaffold platform and then climbing onto the platform, that was over thirty-one (31) feet high, by directly climbing over on the outside of the platform.
