At a 24-volt Network Cable Junction Box located nearest the floor, in the Car Wash Tunnel's Front Right Side area, for the facility located at 1734 Coppergate Drive, Middleburg, Florida: On or about August 06, 2025, the employer exposed employees to electrical shock hazards, in that a knockout-tab from the box's top corner, was not closed (was missing), as employees worked and transited the area.  b.
