29 CFR 1910.178(q)(1):Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: At the jobsite located at 3505 Manchester Trafficway, Kansas City, MO 64129, employees were exposed to crushed by hazards while operating a forklift that was not in safe operating condition. Employees were observed utilizing a forklift that did not have an operational horn and with tries that were missing large chunks.
