29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Pretium Packaging, LLC - Leipsic, Ohio: On or about August 4, 2025, the employer failed to ensure that employees clearing obstructions at the punch station on the Akei Line 7 extrusion blow molding machine, were protected from caught between hazards. The energy control procedures developed by the employer did not ensure include verification to account for the time delay for the release of pneumatic energy that can cause the take-off arm to lower, which created the caught-between hazard.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Pretium Packaging, LLC - Leipsic, Ohio: On or about August 4, 2025, the employer failed to ensure that employees clearing obstructions at the punch station on the Akei Line 7 extrusion blow molding machine (EMB), were protected from caught-between hazards. The established procedure did not account for the release of pneumatic energy that caused the take-off arm to lower, creating a caught-between hazard. The employer did not ensure energy control application steps were implemented to control the machine's pneumatic energy source prior to the servicing tasks; specifically, the employer did not ensure that the following steps were taken: - Turn off or shut down the equipment. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and de-energization of the machine or equipment had been accomplished.
