The employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and guardrail supports. Employees conducting masonry activities were utilizing a scaffold platform that was not fully planked, thereby exposing employees to fall hazards or approximately 20 feet. 29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  The scaffold frame was used as a means of access.   a) Jobsite located at 1801 Webster St. Philadelphia, PA 19146. The employer allows employees to use the scaffold frame on a tubular welded frame scaffold as means of access. Employees were climbing the scaffold frame to access the scaffold to conduct masonry activities. Employees were exposed to a fall hazard of approximately 20 feet in height to the ground level below.  Condition was noted on or about August 4, 2025.

Three employees engaged in masonry activities were utilizing a scaffold platform that did not have guardrail systems along the open sides and ends of the working platform without the use of fall protection, thereby exposing employees to fall hazards of approximately 20 feet.
