29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about August 7, 2025, during a residential construction roofing project located at 6154 Crestview Rd., Seven Hills, OH 44131, employees were exposed to a fall hazard of approximately 9 feet while performing roofing activities on a residential home. Employees were using an extension ladder to access the roof, and the employer did not ensure the extension ladder extended at least 3 feet above the upper landing surface.
