Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On or about: August t, 2025 Location: 45 S Goodwin Ave. Elmsford, NY 10523  a) CSHO observed multiple employees performing work on a roof with cones and flags near the edges acting as a Control Access Zone (CAZ). Two employees were observed working on the other side of the CAZ, by the edge, without fall arrest harnesses. A Safety Monitor was not present. Workers exposed to fall hazard.
