29 CFR 1910.141(b)(1)(i): Potable water was not provided in all places of employment, for drinking, washing of the person, cooking, washing of foods, washing of cooking or eating utensils, washing of food preparation or processing premises, and personal service rooms: Various worksites located in Northeast Connecticut: On or about June 23, 2025, employees who worked on tree trimming crews were not provided with potable water. Employees worked outdoors in hot conditions.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours: Crew Meeting Area, located at 1 Unity Drive, Moosup, Connecticut: On or about September 8, 2025, the employer had not provided copies of the OSHA 300 Log of Work-Related Injuries and Illnesses, including the OSHA 300 Summary form. The injury and illness records were requested via email on August 7, 2025 and were not provided until September 19, 2023.
