29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): (a) On or about July 31, 2025, employees were exposed to a caught in and amputation hazard from the plunger assembly and valve body when the air line was reattached to the valve (in order to move the plunger into the open position) so it could be reinstalled in the CIP system. Employee(s) were not verified to have been removed from the hazardous operation area of the plunger valve assembly prior to reenergizing.
