29 CFR 1926.502(d)(8):Horizontal lifelines shall be designed, installed, and used, under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintains a safety factor of at least two. On or about August 7, 2025, at an establishment located at 109 Wallace Street in Combined Locks, Wisconsin, employees of Security-Luebke Roofing, Inc. were performing residential roofing activities and were utilizing a designed horizonal lifeline with an incapable anchor point capable of supporting 5000lbs while exposed to fall hazards of over 17 feet 10 1/2 inches. There were two employees that were connected to a horizontal lifeline connected to anchors that were only designed for one employee.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: On or about August 7, 2025, at an establishment located at 109 Wallace Street in Combined Locks, Wisconsin, employees of Security-Luebke Roofing, Inc. were performing residential roofing activities and were not utilizing a designed anchor point capable of supporting 5000lbs while exposed to fall hazards of over 17 feet 10 1/2 inches. There were two employees that were connected to a horizontal lifeline connected to anchors that were only designed for one employee.

29 CFR 1926.501(b)(10): Roofing work on Low-slope roofs.�Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted. On or about August 7, 2025, at a worksite located at 109 Wallace Street in Combined Locks, Wisconsin, employees were exposed to fall hazards of 17 feet 10 1/2 inches while conducting residential roofing activities on a low slope roof without the use of fall protection system. Security-Luebke Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1628753, citation number 1, item number 1b and was affirmed as a final order on April 17, 2023, with respect to a workplace located at 4314 N Bull Rush Drive, Appleton, Wisconsin. Security-Luebke Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1609109, citation number 1, item number 2 and was affirmed as a final order on September 28, 2022, with respect to a workplace located at 1570 N. Waterford Drive, Appleton Wisconsin.
