The authorized employee was only locking out the emergency stop at the motor control and was not affixing a lockout device to the energy isolation device.  b.) Brownlee Lumber Inc. Waste Shaker Area, 2652 Hazen Richardsville Road Brookville, PA 15825: On or about 08/07/2025, and at times prior thereto, employee was observed performing servicing and maintenance on equipment including, but not limited to, the Waste Shaker machine. The employee was laying under the rotating shaft while clearing 6x6 board jam.    ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
