29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:  Pole Manufacturing and Sawmill Buildings - The employer did not ensure that working and walking surfaces were maintained in a clean and orderly fashion in the buildings, as debris, piles of bark, metal strapping, wooden and metal poles, and lumber created slip, trip, and fall hazards.

29 CFR 1910.22(a)(2): The employer did not ensure that the floor of each workroom is maintained in a clean and, to the extent feasible, dry condition. The employer did not ensure that when wet processes are used, drainage is maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided:  Sawmill - The employer did not ensure that the area around the trim saw in the sawmill building was kept in a clean, orderly, and sanitary condition. Accumulations of wood dust around the equipment were allowed to build up over a week of processing, resulting in large accumulations that created a combustible dust hazard.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  Planer Area - The employer did not ensure that audiometric testing was made available to employees exposed to noise levels equal to or exceeding 87 decibels (measured on the A scale), while performing de-barking, machine planning, cutting and trimming of logs and boards.

29 CFR 1910.95(i)(5): The employer did not ensure proper initial fitting and supervise the correct use of all hearing protectors:  Planer Area - The employer did not ensure that employees were correctly wearing hearing protection. Employees were wearing earmuffs over hoodies, using insufficiently inserted earplugs, and using headphones as hearing protection, which compromised the hearing protector's effectiveness to provide hearing protection.
