29 CFR  1910.1200(e)(1):Employer had not developed or implemented a written hazard communication program that included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):   At a worksite located at 4701 SE Claude Ct, Blue Springs, MO, employees were exposed to health hazards.  The employer exposed the employees to these hazards by allowing them to work with materials that include, but are not limited to, "Titebond Subfloor Construction Adhesive" without first establishing a written hazard communication program that addresses the hazards associated with the materials used by employees.  Hazards related to the use of "Titebond Subfloor Construction Adhesive" include but are not limited to:  Highly flammable liquid and vapor, Causes skin irritation, suspected of damaging fertility or the unborn child, may cause respiratory irritation, causes damage to organs (CNS, kidneys, liver) through prolonged or repeated exposure if inhaled.

29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  At a worksite located at 4701 SE Claude Ct, Blue Springs, MO, employees were exposed to struck by hazards to their heads.  The employer exposed the employees to these hazards by allowing them to work without head protection underneath employees at higher levels.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At a worksite located at 4701 SE Claude Ct, Blue Springs, MO, employees were exposed to fall hazards.  The employer exposed the employees to these hazards by allowing them to perform framing work at heights greater than 6 feet without the use of fall protection.  Instances of exposure include but are not limited to:  Instance 1:  Two employees were exposed to fall hazards greater than 19' from the level they were working on to the next lower exterior ground level.  Instance 2:  Two employees were exposed to fall hazards greater than 8' from the level they were working on to the next lower interior ground level.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a worksite located at 4701 SE Claude Ct, Blue Springs, MO, employees were exposed to fall hazards.  The employer exposed the employees to these hazards by allowing them to utilize a portable extension ladder while the side rails of the ladder did not extend at least three feet above the level for to which it was being used to gain access.
