29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a.) Accommodative Construction Services LLC, 11389 Vernon Place, Meadville, PA 16335: On or about 08/06/2025 and at times prior thereto, Employees were exposed to fall hazards of approximately 20 feet to ground level while working off the 3rd buck of the frame scaffold at the front of the future shoe retail store. Employees were not protected by a complete guard rail system or a fall protection system. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Accommodative Construction Services LLC, 11389 Vernon Place, Meadville, PA 16335: On or about 08/08/2025 and at times prior thereto, Employees were exposed to fall hazards of approximately 15 feet to ground level while working off the 2nd buck of the frame scaffold at the front of the future shoe retail store without a complete guard rail. A mid-rail was not used below the cross bracing on the 2nd buck of the frame scaffold. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
