Employees were working directly below the overhead work without wearing protective helmets; on or about 8/8/2025. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

The employees were not using any means of fall protection and were exposed to falling approximately 18-feet to the ground below; on or about August 8, 2025. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
