29 CFR 1926.652(c)(2)(i):Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data shall be in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer. On or about August 8, 2025, during the replacement of a water line at a worksite located at the corner of South Elmwood Avenue and West Washington Street, Medina, Ohio, the employer failed to ensure a trench shield was used in accordance with all of the manufacturer's tabulated data specifications, recommendations, and/or limitations. A GME 2AEX68 Aluminum Extruded Trench Shield was installed in an approximately 7 foot deep trench in a vertical orientation on the trench shield end wall and without pins placed in all of the spreader collar positions.

29 CFR 1926.652(g)(1)(iii):Employees shall be protected from the hazard of cave-ins when entering or exiting the areas protected by shields. On or about August 8, 2025, during the replacement of a water line at a worksite located at the corner of South Elmwood Avenue and West Washington Street, Medina, Ohio, the employer failed to ensure employees working inside a trench, with a depth of approximately 7 feet, were protected from a cave-in hazard when entering or exiting the area of the trench protected by a trench shield.
